Mysterious Mix Master is a Coiled Isle exploration achievement in the Curse of Ula’tek update, and it comes down to one repeatable daily and a fixed set of recipes. You feed Ofi the Sly three potent ingredients at a time, and the exact mix you hand over decides which offering pops out of the cauldron. Collect all of them and the achievement is yours in a single sitting.

The whole thing runs through Mixing Mysteries, a daily quest you pick up from Ofi the Sly, who stands right beside Ofi’s Cauldron at Tokka’s Landing on the Coiled Isle. The quest requires level 90, and it hands you a menu where you offer three “potent” ingredients to the pot.
Buy all 30 ingredients (10 of each) from the Auction House before you talk to Ofi — with a full stock in your bags you can chain every offering back-to-back and finish the achievement in one visit.

Making the tenth offering unlocks Mysterious Mix Master, and the reward is the Mysterious Voodoo Mask decor item for your housing collection. Progress on the achievement is shared account-wide, so any character can chip away at the list.
If the completion popup doesn’t fire after your last turn-in, open the in-game achievement panel and compare the offerings it’s tracking against the ones you’ve already made — that tells you at a glance which combination still needs handing to the cauldron.
