How to get Mether’s Flesh in Mortal Shell 2
Your first objective is to obtain the Mether’s Flesh key item. Travel to the Cleansed Gate beacon in Sanguine Caverns. The surrounding environment is immediately recognizable by its organic, flesh-covered appearance, so you should have little trouble identifying the correct area once you reach the beacon.
Step 1
From the Cleansed Gate beacon, head down into the cavern. You can fight the enemies along the route or avoid them if you are only interested in reaching the item.

Step 2
Continue deeper into the flesh-lined cave and look for the gaps that allow you to drop down into the lower sections.

Step 3
Work your way through the cavern until you find an opening leading into a more enclosed room. Inside is an altar-like area containing a sarcophagus or chest.

Step 4
Interact with the sarcophagus/chest to obtain Mether’s Flesh. This is the important item required for the next part of the secret, so make sure you have picked it up before leaving the area.

How to offer Mother’s Flesh at the Outskirts of Nochte
The second part of the route takes you to the Outskirts of Nochte beacon. This is where Mether’s Flesh becomes the key to reaching the otherwise hidden area containing the Mango.

Travel to the beacon and look for the nearby ruined structure and archway. From there, the route leads toward a stream and eventually into a cave.

From the Outskirts of Nochte beacon, move through the archway and ruins.

Step 6
Follow the stream until you reach the cave opening ahead.

Step 7
Keep moving until you reach a set of altar-like platforms overlooking the water. The area is marked by torches positioned around the platforms, making it easier to recognize.

Step 8
Approach the altar and interact with it. Select Offer Item, then choose Mether’s Flesh from your inventory.

This interaction is the trigger that opens the path to the Mango. A huge creature rises from the water after you offer the item and swallows the Harbinger. Although the sequence can look alarming the first time you see it, this is the intended progression for the secret.
How to collect the Mango and unlock Ascension trophy
After the creature swallows you, you will wake up in a new location. This is effectively the transition into the hidden area required for the Ascension trophy.

Continue through the tunnel until you reach the Mango Sanctuary. This is the hidden destination that contains the secret referenced by the Ascension trophy description.
Inside the Mango Sanctuary, look for the large golden mango positioned in front of the red tree. Approach the golden mango and interact with it when the Pick Up prompt appears.
Pick up the Mango. The moment you collect it, the Ascension trophy/achievement should unlock.

How to avoid missing the Mango secret
The most important thing is not to confuse the Cleansed Gate and Outskirts of Nochte portions of the route. The first location gives you Mother’s Flesh, while the second provides the altar interaction that transports you toward the Mango Sanctuary.
You also do not need to defeat a major boss as part of this secret. The unusual creature encounter is effectively the transition mechanism for the area. Once it swallows you, simply follow the new cave route until you reach the sanctuary.
