Head east from the tavern
Cross the rope and wooden bridges, keeping east or northeast along the cliffside route.

Enter the Bone Gate/Traverse Gate to cross the chasm and reach the island holding the Temple of Vatra.

Climb through the temple
Enter the ruins, stay on the main path, and follow the stairs toward the highest part of the complex.

What changes after taking the Heart of Vatra
Picking up the Heart causes the nearby petrified cultists to awaken and attack. They can quickly surround you, so be ready to fight or move as soon as the pickup animation finishes.
The gate on your original route can also become inaccessible. If that happens, check the right side of the blocked gate; a side route may loop toward the Berserker’s Stone area and provide another way out without forcing you through every awakened enemy.

Return to Gragu
Travel back to the One Legged Wolf Tavern and find Gragu sitting inside.

Hand over the Heart
Speak to him and choose Yes when he asks for the Heart of Vatra.

Claim Gragu’s Shell
After the cutscene in which Gragu eats the Heart and dies, inherit his body as a playable Shell.

Light all three candle offerings and open the Berserker’s Stone chest before leaving the Temple of Vatra.
