First, visit the Proactiv Contact Us page and put down your account information, which would allow the customer care representatives to fetch your information. Now, under ‘Comments/Questions Concerns,’ select the option ‘Account Question.’ Then, under the ‘Message’ banner, type “I want to cancel my membership.’

Someone from the Proactiv team would get in touch with you in a short while. 

Method #2: Through phone call 

Proactiv has listed a toll-free number that you can use to get in touch with the concerned team. Dial “1-800-309-4796” to connect to Proactiv and eventually talk to a customer care representative.
