Perfect Predator is a timing achievement, not a full hunt. The mistake is treating any clean shark hit as enough: the game wants the perfect-charge cue on a Great White Shark harpoon encounter, and the unlock can happen before the animal dies.
Perfect Predator requires a Great White perfect shot

KEY!The requirement is literal: get a perfect harpoon shot on a Great White Shark. A normal shark, another shark type, a whale, or any other sea animal will not satisfy this achievement even if your throw looks clean.
A perfect shot is a charged harpoon hit released at the full-charge pulse/flash, when the reticle is at its smallest. You only need one of those hits to connect, and Perfect Predator should pop as soon as the harpoon lands.
Do the attempt on a Great White Shark marker first; the timing can be perfect and still fail the achievement if you are hunting the wrong animal.

The simplest route is to sail the Jackdaw to the far southwest side of the world map, near the southern edge, and uncover the small area with the Great White Shark harpoon marker. You can travel there even before every surrounding map point has been discovered; once the marker is revealed, sail into it and start the activity.
Great White Shark activities use the standard white harpoon icon on the ocean. The map can point you to more than one southern Great White spot, so if one marker is not visible, sweep the southern edge and check the alternates below.
Once the marker is active, avoid starting during a storm; the activity can be unavailable until the weather clears. Clear weather and a visible harpoon marker are enough to begin the rowboat encounter.

The buttons change by platform, but the timing does not. Hold the throw long enough for the reticle to shrink and the charge effect to pulse, then release right on that pulse/flash.
If you are seeing the harpoon leave Edward’s hand immediately, you are tapping the throw instead of charging it. Keep the shark or its head in the reticle while the charge builds, then release rather than trying to correct after the pulse has already passed.
Why Perfect Predator does not unlock
The most common failure is using the wrong target. Perfect Predator specifically checks for a Great White Shark, so a perfect-feeling throw on another shark type or sea animal will not count.
The second issue is the charge. If you tap R2, RT, or the mouse input, you throw a normal harpoon. If you hold too briefly or release after the pulse is gone, the hit may still damage the shark but will not be the perfect shot.
Weather can also block the attempt. If the harpooning activity will not start, wait for the storm to end or leave and return; harpooning is unavailable during storm conditions.
Finally, aim at the shark, preferably the head/mouth line when it surfaces, not just the disturbed water. Splashing water tells you where the Great White is moving, but the harpoon still needs to connect with the animal for the achievement to fire.
