At San Juan, do not waste time sweeping the outer beach. The useful entrance is offshore near the broken boat area, where the jellyfish lead into an underwater tunnel that runs toward the cave.
Once you surface, the plan is close by. Clear the first guards, then look near the right side of the ramp or just before the wooden scaffold. The dirt patch there gives the Dig Up prompt for the buried chest.
Collect the Santanillas map before searching San Juan; the cave route matters, but the buried plan route starts with the map at 191, 218.
