The item you need is Seedbearer’s Scripture, the game’s dedicated counter to Cosmic Disease. Cosmic Disease deals stacking damage over time while lowering your outgoing damage, punishing both your health and offense at once. The Scripture is especially valuable against the final boss, Zmey, the Unbidden, where that double penalty can quickly end a run.

Where to grab the Depleted Gland
Travel to Withered Shoals using the Nameless Pass Beacon, the closest fast-travel point to the Gland. From there, find the bone arm propped beside a lit torch. The Gland is held at that spot and sits in the open once you locate the correct torch. This is the most straightforward pickup, so clear it first.
Reaching the Thawed Gland

The Thawed Gland is on the Corrupt Gate route near the abbey approach to the Valley of the Revered. Follow the route into the statue chamber, the same room containing Martyr’s Ash, and search the chamber for the Gland. It is less exposed than the other two pickups, so sweep the entire room instead of looking only in the open.
Locating the Faded Gland
The final pickup, the Faded Gland, is in the Faded Capital near the Citadel Annex Beacon. Enter from the beacon and find the stone, well-like dish ringed by candles. The Gland rests inside it. Use the ring of candlelight as your landmark, since it is easier to spot than the dish itself.

Reach the top of Marrow Keep

Climb to the highest point of the hub and approach the NPC who controls the world’s day-night state.
Speak with Thestus

Talk to Thestus and switch the world to Night Mode. The offering becomes available only at night.
Fast travel to Sester’s Beacon

Once Night Mode is active, fast travel to Sester’s Beacon to begin the trip to the tower.
Head west to the Forgotten Tower

Travel west from the beacon to the separate island containing the Forgotten Tower, then go inside.
Climb to the upper floor
Climb through the tower to the top level, where the offering corpse is waiting.
Use Make Offering on the corpse

Select the Make Offering prompt and hand over all three Glands. You will receive Seedbearer’s Scripture.

Seedbearer’s Scripture has two different effects depending on the slot you use, and both are useful against Zmey. As of Balance Patch 1 (August 20, 2026), its active use cures Cosmic Disease and grants 50% mitigation for 30 seconds, while its passive effect provides 15% mitigation at all times.
Older figures list much higher tiered values, including full mitigation for two minutes, but those values do not apply to the current build. Plan around the values above.
Save the Scripture’s active use until Zmey lands a Cosmic Disease attack. Activating it then briefly staggers him and creates a free punish window without wasting the cooldown early.
Mistakes that block the reward

Traveling to the Forgotten Tower during daylight blocks the reward because the offering prompt will not appear until you activate Night Mode. Before fighting Zmey, equip the Scripture in the active slot; leaving it unequipped or using only its passive effect prevents you from curing the status when you need it. Zmey, the Unbidden is the only enemy that inflicts Cosmic Disease, so reserve the Scripture for this boss instead of building your entire run around general-purpose disease resistance.

Why does the Forgotten Tower offering prompt not appear?
The prompt appears only during Night Mode while you are carrying all three Glands. If night is already active, check that you have the Depleted Gland, Thawed Gland, and Faded Gland, then climb to the corpse on the tower’s upper floor. The interaction is not at ground level.
Does Cosmic Disease resistance fully prevent Cosmic Disease?
No. The Scripture mitigates Cosmic Disease rather than granting immunity. You can still contract the status while it is equipped; its resistance reduces the damage, and its active use provides a safer combat window. Continue dodging Zmey’s disease attacks instead of standing in them.
