Know-It-All is tied to reading every Document, but the commonly shared 22-Document count does not yet come with a full named location list, so use this as a cleanup tracker rather than a finished location map.
The important part is simple: Know-It-All is a Document-only trophy in Beast of Reincarnation. It is not advanced by reading shrine text, finding campsites, discovering lore paintings, collecting manuals, gathering blades, or clearing other collectible sets.

Know-It-All requires you to read every Document. Picking one up is not enough if the game separates collecting from reading, so open and read each Document as soon as you find it.
The trophy is listed as a silver trophy with the condition “Read every document.” Treat Documents as their own checklist while you work through the rest of the map, because several other collectible trophies sit beside it but do not feed into this one.

Check side rooms, ruins, village spaces, and dead ends before pushing the main objective forward.
Read every Document on pickup

Open each Document right away so the trophy progress is tied to a completed read, not just a collected item.
Revisit side areas as paths open

When new routes, gates, or traversal options become available, sweep older areas again for missed interactables.
Combine cleanup with other collectibles

During the campaign, gather blades, seedlings, manuals, cooking books, crop books, and other sets when they are nearby.
Use post-game free-roam

After the ending, revisit regions, shrines, and campsites before moving into New Game+.
Finish your Document sheet

Compare your own list against the 22 slots above and recheck any blank entries in the same save.
KEY!
Know-It-All only cares about Documents. Beast of Reincarnation has several other completion goals, so mixing them together is the easiest way to think you are closer to the trophy than you actually are.
Those other sets are still worth collecting during the same pass, especially if you are aiming for 100%, but they should be tracked separately from Documents.
New Game+ timing
Before starting New Game+, finish collectible cleanup in your current save. Post-game free-roam lets you revisit areas, which makes it the better moment to fill in missing Document slots and finish nearby collectible work.
There is no current sign of a New Game+-exclusive Document, and this guide should not be read as saying Documents are permanently missable after the ending. The main risk is losing track of what you already read if you roll forward before cleaning up your active save.
