Diving Bell access requirement
La Concepción Wreck is a shipwreck dive site, so you need the Diving Bell before the chest can be reached. Progress the main story through Sequence 06 – Mission 01: Diving for Medicines; after that point, shipwreck dive activities become available.
Once the Diving Bell is available, travel to the wreck and start the underwater activity from the bell. The Ram Ultimate Plan route begins from that starting point, not from a surface chest or a shop menu.

Looting the chest adds the Ram Ultimate Plan to your upgrade pool; it does not attach a new ram to the Jackdaw by itself.
Buy the final ram tier from a Harbourmaster when you have the required Reales and materials. If Ram IV is missing from the list, improve the Great Inagua Harbourmaster/shop first; the dockside shop upgrades make the top-tier ultimate upgrades appear at Harbourmasters.
Common Ram Ultimate Plan mistakes
Do not sail to Chinchorro early and expect to force the chest. The blocker is the underwater activity itself, so the Diving Bell comes before the wreck route.
Do not rely only on coordinates if they pull you away from the named wreck. The La Concepción Wreck map icon in Chinchorro is the target.
The easiest underwater miss is circling the wreck from outside. The chest is inside the broken northern ship section near the jellyfish, on the right after you enter.
The plan is also not the upgrade purchase. If your ram is unchanged after looting the chest, nothing broke; the plan only opens the final ram upgrade path at the Harbourmaster.
