Clear the three Wraiths by the gate
Turn right toward the closed iron gate and deal with the three Wraiths guarding it before pushing on.

Go left past the gate to the room with the giant skeleton, then approach the body slumped in its ribcage to take the Immaculate Crown.
Do not leave right away
