The update also adds quality-of-life changes aimed at faster-paced gameplay, smoothing out hospital runs once you know the route. New Doctor, Surgeon, and Paramedic skins round out the update.

The Rain Shift and its aquatic patients

The new special shift is the Rain Shift. Complete Shift 3 to add it to the possible shift rotation. It then triggers randomly, and there is no way to force it before that point.
When the Rain Shift begins, rain pours down and water leaks into every hospital room, including the reception area. The flooding changes the patient list, bringing in aquatic species, including an Anglerfish. This disruptive, anomaly-heavy encounter is the lore update’s “new shift.”
Common Rain Shift mix-ups to avoid
Two details cause most Rain Shift confusion. First, the Rain Shift is different from the older Hazmat / slime special shift. That shift arrived in a separate July update and uses different triggers.
Second, the Rain Shift cannot start during Shift 1 or 2. It enters the rotation from Shift 3 onward, so clear the opening shifts before looking for the rain event.
The Rain Shift only becomes possible from Shift 3 onward, so push past the opening shifts before deciding it’s missing or bugged.
