When and where this objective appears
Headaches normally starts once you finish the Fated Land Myth in the Warring States chapter, after which Tokugawa Ieyasu asks for help with his recurring headaches.

Enter Hamamatsu Castle. Climb to the upper floor where Tokugawa stands near the balcony.

Speak with him to trigger the quest to seek medicine along Futamata Road.

From that point you are directed straight toward the ruined village near Futamata Road, where the main task is to enter a particular house and inspect the healer’s corpse to reveal the medicine clue.

Futamata Road stretches out from the area around Silver Grass Meadow into a narrow rural stretch tangled with yokai‑twisted trees and broken buildings.

Open the door and walk inside, moving toward the back or far end of the room where a corpse rests against the wall or resting platform.

You must chase it toward the courtyard’s edges until it stops fleeing and turns hostile in a corner. Defeat it in this position to obtain the Yokai Gallstone it drops, then head into the Eternal Rift. At the Rift forge, open the Forge Tools menu and use the recipe you found on Futamata Road to craft Ghostly Gall.
With the crafted medicine in your inventory, return to Hamamatsu Castle, deliver Ghostly Gall to Tokugawa, and talk once more with Hattori Hanzo to wrap up all quest markers and receive your rewards.
