When and where this objective appears
Headaches normally starts once you finish the Fated Land Myth in the Warring States chapter, after which Tokugawa Ieyasu asks for help with his recurring headaches.
Enter Hamamatsu Castle. Climb to the upper floor where Tokugawa stands near the balcony.
Speak with him to trigger the quest to seek medicine along Futamata Road.
From that point you are directed straight toward the ruined village near Futamata Road, where the main task is to enter a particular house and inspect the healer’s corpse to reveal the medicine clue.
Location and layout on Futamata Road
Futamata Road stretches out from the area around Silver Grass Meadow into a narrow rural stretch tangled with yokai‑twisted trees and broken buildings.
After fast‑travelling to the Silver Grass Meadow shrine, move south along the main road and you will come upon a small, decaying village where most houses have collapsed or been strangled by spiraling tendrils.
Among this wreckage, one wooden house still stands mostly whole by the roadside; this is the structure you need to search. The surrounding paths host smaller yokai, so it helps to clear or avoid them before entering the building to focus on your objective.
How to find the medicine clue in the town
Step 1
Reach the village section south of Futamata Road by fast‑travelling to the Silver Grass Meadow shrine, then heading down the southern‑facing main road into the area overrun with twisted trees.

Look carefully at the structures that line the road; one house near the shrine‑side stretch will remain intact compared with the ruins around it.
Step 3
Open the door and walk inside, moving toward the back or far end of the room where a corpse rests against the wall or resting platform.
Step 4
Interact with the body to open its item window; from there, take the Craft Guide: Ghostly Gall, which is the medicine clue described in your objectives.
Step 5
With the recipe secured, exit the house, clear a safe path if needed, and fast‑travel back to Hamamatsu Castle to continue the quest.
What happens in the quest afterward
Once you return to Hamamatsu Castle with the Ghostly Gall craft guide, you speak first with Tokugawa and then with Hattori Hanzo, who warns that yokai have begun pressing against the castle grounds. Near the East Gate, a Nuppeppo yokai rolls around and initially runs away when attacked.
You must chase it toward the courtyard’s edges until it stops fleeing and turns hostile in a corner. Defeat it in this position to obtain the Yokai Gallstone it drops, then head into the Eternal Rift. At the Rift forge, open the Forge Tools menu and use the recipe you found on Futamata Road to craft Ghostly Gall.
With the crafted medicine in your inventory, return to Hamamatsu Castle, deliver Ghostly Gall to Tokugawa, and talk once more with Hattori Hanzo to wrap up all quest markers and receive your rewards.
