Complete a short combat and torch-lighting puzzle at the in Echoes in the Shrine quest to unlock the lower level and retrieve the document.

Read the document in your inventory to register it. Return to General’s Shrine, break the cracked stone slab near the entrance to reach the underground.

Progress through the dungeon to the final ladder and open the hidden chest beside it to unlock the antique.

Order does not matter. You can open the chest first and read the document later, or read the document first, then open the chest.
